Community Championed Home Hospice Program

Overview:

A community wellness collaborative identified the need for more palliative care and funded and provided volunteers for a home hospice program.

Then the most recent thing is around the palliative care initiative, when we went from having just us doing palliative care with the system and now, because of the Collaborative getting together, identifying it and creating a whole process to assess it, we now have a full home hospice program developed with volunteers already trained and a coordinator and a whole thing happening. Where a year ago we had like no community services for hospice, we now actually have an active organization with funding to be a hospice on the community. But that was identified by the community. And we didn’t hear that as in 'You doctors aren’t providing good enough palliative care work.' we heard that as 'We need to do something more for our families and patients.' and then we did it and it’s happened.
