Overview:

Licensed Practical Nurses (LPNs) assist to support smooth and safe transitions from hospital to home for patients.

[We] just created the Licensed Practical Nurse (LPN) Rapid Response Team. They’re the folks who when they have a familiar face coming through the ER who has COPD or frailty, the LPN will take the patient home, set them up there, get their medications, and refer them to home support rather than having them admitted. [It's] Just getting off the ground here, all the LPNs have come from rotary manor.
